IMPORTANT INFORMATION REGARDING INSURANCE COVERAGE - PLEASE READ

We are currently in-network with the following insurance companies:

Aetna
Blue Cross Blue Shield (BCBS) of Texas
Humana (LifeSynch)
Magellan
Tricare/Humana (for active-duty military familes)
United Behavioral Health

If you have insurance through one of the companies listed above, we will request benefit information on your behalf and contact you to discuss the information your carrier provides. If you are insured through Tricare/Humana, please see the next paragraph. We will be unable assist you if the information provided in the form below is not complete and accurate. Please note that the legislation in the state of Texas only pertains to insurance policies that are fully-funded. Self-insured insurance policies are governed by the federal government. Although some self-insured insurance policies cover Applied Behavior Analysis (ABA) therapy, there is no requirement by the federal government that requires them to do so.

TRICARE/HUMANA FAMILIES - Your provider requires you to initiate the insurance coverage process. After you fill out the form, please call Humana at 800.447.8808. After you hear the voice prompt, press 1 if your last name begins with A-L or 2 if your last name begins with M-Z. Your case manager will assist you with the paperwork and set up an assessment for your child.

IF YOUR INSURANCE COMPANY IS NOT LISTED ABOVE, YOU WILL NEED TO CONTACT THEM TO DISCUSS YOUR CHILD'S BENEFIT INFORMATION. After you understand your coverage and out of pocket costs and are interested in discussing enrollment, please send an email to info@behavioral-innovations.com with "Out of Network" in the title. Please include your insurance company and benefit information along with your phone number and the best time to contact you during the day.
